Lost or Stolen License Plate?
If the license plate on your vehicle is valid (not expired), you may obtain another plate with the same
classification and expiration date at your local Vehicle and Plate Renewal Office.
If you just want to report your License Plate lost or stolen, you will need to stop by your local
Vehicle and Plate Renewal Office.
The Steps to getting your new license plate are:
- Complete the form
Application for Replacement of Plate and/or Sticker (MVR-18).
- The fee is $15.00, plus a notary fee and you will need to advise them of your insurance company name and
policy number.

- Print and complete the form
Application for Replacement of Plate and/or Sticker (MVR-18)

- The form must be completed listing the vehicle information and must be signed and notarized.
- The MVR-18 form and $15.00 fee should be mailed to:
