Issue Details:  
Annually, contractors are required to submit information on the FHWA-1391 form for Federal Aid projects, to the Resident Engineer’s office. This form is now available in an excel spreadsheet on the electronic forms listing. PR-1391 is the spreadsheet and PR-1391UG is the user’s guide.
