Form
- A printout form will be used for the pavement condition survey. Each section of road was set up with mileposts and descriptions from previous surveys.
- A brief review of the form is needed to explain some of the abbreviations.
- CO/CITY CODE: This denotes the County and City in which the section is located. There is no City Code for sections outside the city limits. City and County codes will be provided.
- ROUTE: The sections are listed with the US routes first, followed by NC routes and then Secondary routes. They are listed numerically from lowest to highest. The routes are coded as explained on the next page.
The route is a five-digit code broken down as follows:

For divided highways, each direction of travel shall be rated as a separate section and coded with the letter, either (N) North, (S) South, (E) East, or (W) West.
- BEGIN MILEPOST: This indicates the milepost of the beginning point of the section.
- BEGIN DESCRIPTION: Brief description of the beginning point of the section.
- SECTN LENGTH: Length of the section.
- END DESCRIPTION: Brief description of end point of the section.
- PAVMNT (Pavement)
- T = Type, either P(PM), B(BST), S(Slurry), U(Unpaved), or C(Concrete);
- WI= Width, excluding any paved shoulders;
- L = Number of through lanes, including continuous center left turn lanes.
- SLDR (Shoulder)
- T = Type, either P(Paved) or U(Unpaved);
- WI= Width (average for section); paved shoulder must be continuous on both sides.
- C & G (Curb and Gutter)
- Y = Yes, must be continuous on both sides
- N = No, including one side only.
This information should be checked by the rater and corrections made as needed. Changes should be made by marking through incorrect data with one line and writing correct data above the marked out data.
- All the distresses are then rated (see Section J for definitions and photos):
- ALLIGATORCRK = Alligator Cracking with the percent of the section rated for the four severity levels of N = None, L = Light, M = Moderate, and S = Severe. Rate the percentages as 01 (10%), 02 (20%),....10 (100%).
- TRV = Block/Transverse/Reflective Cracking, rated with one letter of either N = None, L = Light, M = Moderate, or S = Severe.
- RUT = Rutting, rated with one of the four letters, (N,L,M,S).
- RAV = Raveling, rated with one of the four letters, (N,L,M,S).
- OXD = Oxidation, rated with one of the four letters, (N,L,M,S).
- BLE = Bleeding, rated with one of the four letters, (N,L,M,S).
- RID = Ride Quality, rated with one of the three letters as follows:
L = Average, M = Slightly Rough and S = Rough.
- PAT = Patching, rated with one of the four letters, (N,L,M,S).
Pavement distresses should be evaluated independent of the 2000 distress information. It is possible that the pavement condition in 2000 is significantly different from the condition seen in 2002. Each rater will be given forms with sections shown from the 2000 survey but all of the distress information will be left blank.
