Structure Design Unit's Submittal Forms for Proposed Heavy Equipment on Bridge
When the Contractor is proposing to place heavy equipment on a new bridge deck, the proposed plan shall be submitted to the Engineer, in accordance with the Standard Specifications, Section 420-20.

The Contractor is not required to use these submittal data forms for any submittal. The submittal data form is only an aid in making a complete submittal.
