1. Download electronic file to company computer.
   
   
2. Input attendees’ information, replace sample name on first line with the company’s first attendee’s information. Badge name is the name that the attendee likes to be called.
   
   
3. Save and then email electronic file to pseymore@ncdot.gov
   
   
4. Make check payable to: "Signing and Delineation Unit"
   
   
5. Print a hard copy of form and mail it with a check to:
